To conduct a comprehensive review, we require access to the applicant's previous medical reports and forms. This allows us to identify which conditions are new and which are pre-existing, ensuring a thorough and accurate assessment. By doing so, we help safeguard the integrity and depth of the review process.

For the most complete evaluation, we recommend that as much health information as possible be provided at the time of review. Ideally, this should include a Form AH2 Adult Health Report or a Form AH – Adult Health Report. This level of detail supports fostering and adoption agencies in making confident, well-rounded decisions during the renewal process.
