GP Medical Assessments:

Examinations for Prospective Foster Carers or Adopters (AH & AH2)

In response to years of requests from our valued clients, we are pleased to introduce a GP-led medical assessment service designed specifically for prospective foster carers and adopters. Initially available in the Northwest of England, this service will gradually expand to cover the entire UK.

Our goal is to provide a seamless, straightforward process from your initial enquiry through to booking and receiving the completed medical forms. If needed, we can also include a medical adviser report as part of the service—please visit our Medical Adviser Services page for more details.

Form AH and Form AH2

We offer both Form AH (Adult Health Report) assessments and Form AH2 (Adult Health Report Reviews). To ensure the service is both safe and effective, access to the individual’s GP medical records is required—please refer to The Process page for more information.

For Form AH assessments, we believe that face-to-face appointments provide the most thorough evaluation of the patient’s health. Unlike remote consultations, in-person assessments allow for a clear understanding of mobility and functional abilities. Additionally, our GPs can obtain accurate, up-to-date measurements such as height, weight, blood pressure, and urine testing, as required by the Form AH. These appointments also enable our GPs to conduct any further examinations they may deem necessary.
