Face-to-Face Appointments

We provide face-to-face appointments where our experienced GPs will complete Part C of Form AH with the applicant after conducting the necessary examination and urine tests as outlined on the form. Applicants must bring or send in advance a copy of their complete GP medical records. A proforma letter for requesting these records is available for download in the applicant section of our website.

Review Appointments

Applicants do not need to be present for review appointments, but a copy of their GP medical records with all entries dated after their last full medical review is required. A proforma letter for requesting these records can be downloaded from the applicant section of our website. Additionally, a copy of their previous medical review and/or relevant forms (provided by the fostering or adoption agency) will be needed.
