A Complete Guide to the Adoption Medical Assessment Process

What’s Involved:
Requires a GP appointment.
Includes a review of your medical information (Form AH - Adult Health Report), your medical records, and a brief physical examination.
The examination involves checking your height, weight, blood pressure, and conducting a urine dip test.
If any abnormalities are found, the GP may perform additional examinations during the appointment or recommend follow-up with your regular GP.

What’s Involved:
Does not require a GP appointment.
Involves a review of your completed medical form and medical records dated since your last assessment.
We will complete the Form AH2 Adult Health Report based on this information.

Medical Records:
A copy of your GP medical records is required for all assessments.

Preparation:
Do not attend your appointment unless you have your medical records with you (either in paper form or on a USB memory stick), or you’ve sent them to your adoption agency to forward to us.

Forms:
Your adoption agency will provide the necessary forms for a new or review assessment.
Ensure your section of the form is completed before your assessment, as the appointment cannot proceed otherwise.

Once all the required documentation is ready, your adoption agency will arrange an appointment for full assessments.
