A Complete Guide to the Adoption Medical Assessment Process

AH Medical (Initial Application)

  • What’s Involved:
  • Requires a GP appointment.
  • Includes a review of your medical information (Form AH - Adult Health Report), your medical records, and a brief physical examination.
  • The examination involves checking your height, weight, blood pressure, and conducting a urine dip test.
  • If any abnormalities are found, the GP may perform additional examinations during the appointment or recommend follow-up with your regular GP.

AH2 Medical (Review for Approved Adopters)

  • What’s Involved:
  • Does not require a GP appointment.
  • Involves a review of your completed medical form and medical records dated since your last assessment.
  • We will complete the Form AH2 Adult Health Report based on this information.

Important Information for All Assessments

  • Medical Records:
  • A copy of your GP medical records is required for all assessments.
  • Click the download link below for instructions on how to request these using a simple form.
  • Preparation:
  • Do not attend your appointment unless you have your medical records with you (either in paper form or on a USB memory stick), or you’ve sent them to your adoption agency to forward to us.
  • Forms:
  • Your adoption agency will provide the necessary forms for a new or review assessment.
  • Ensure your section of the form is completed before your assessment, as the appointment cannot proceed otherwise.
  • Fees:
  • All fees for these services are covered by your adoption agency.
